1 month    HBV #2 (assuming #1 was given in hospital)

2 months    DTaP#1, IPV#1, Hib#1, Prevnar#1, Rotateq#1

4 months    DTaP#2, IPV#2, Hib#2, Prevnar#2, Rotateq#2

6 months    DTaP#3, Hib#3, Prevnar#3, Rotateq#3

9 months    HBV#3 (also Hemoglobin check)

12 months    Prevnar#4, Varicella#1, HAV #1  

15 months    MMR#1, Hib#4 

18 months    DTaP#4, IPV#3, HAV #2

4-5 years    MMR#2, DTaP#5, IPV#4, Varicella#2

11 years   Tdap (Tetanus Booster)

11+ years  MENACTRA Guardasil (Human Papilloma Virus Vaccine) for females only
